Cedar Heritage Trust is pleased to award Lakeshore Literacy Project a grant of $32,000 to support Lakeshore Adult Literacy 2026 as proposed in application APP-2035. This award is made under the Education Innovation program for the grant term Apr 1, 2026 through Apr 1, 2027. Acceptance of this grant constitutes agreement to the terms, conditions, payment schedule, and reporting requirements set forth below. Please countersign and return the enclosed acceptance form within 14 days.
Conditions
Maintain instructor-to-learner ratios as proposed. Submit attendance records quarterly.
Failure to comply with these conditions may result in suspension of payment milestones, request for return of disbursed funds, or termination of the grant. The foundation reserves the right to conduct site visits, request additional documentation, and modify reporting requirements with 30 days notice.

Milestone payments are released via ACH to the grantee's bank of record after each gating condition is met. Payment is held automatically when a grantee report is overdue more than 14 days. Reference numbers appear on remittance advice once releases post.
